Important Notice Regarding Fit Notes

We would like to inform all patients about the process for fit notes:

1. **If You Have Been Treated in a Hospital: **

Cotswold Medical Practice is unable to provide Fits notes for patients who have been discharged from the hospital. If you need fit notes following a hospital stay or treatment, you should request one from the healthcare professional responsible for your discharge. You should discuss your need for a fit notes with them *before* you are discharged. They are the best person to assess your condition and provide the necessary documentation.

2. **If You Have Been Treated in Our Surgery: **

Cotswold Medical Practice can issue fit notes for patients treated directly in the surgery. If you need a fit note due to your health condition, please discuss with a GP or contact reception. A doctor will assess whether your condition impacts your ability to work and whether a fit note is required.

3. **General Guidance on Fit Notes: **

A fit note, also known as a medical certificate or doctor’s note, is only necessary if your health condition affects your ability to work. The healthcare professional treating you will determine if a fit note is needed based on your specific situation.

If you have been ill for 7 days or less, you don’t need to see a doctor. You can complete a Self-Certification form yourself. However, some employers insist on a doctor’s note regardless. A doctor’s note under these circumstances may involve a charge.
