About the Patient’s Problem

If you are unsure of how to word it, it might help to write down the problem as if you were speaking to someone.
If you’re not sure, it’s helpful to tell us whether it’s been days, weeks, months or years.
This might be something that happened or that you discovered around the time your symptoms began.
Think of anything that might be worrying you about the problem you are telling us about today.

Attach photos or other files

Please send photos if your problem is a rash, mole, lump, or if you think a photo would help our clinicians.

Tips on taking good photos:

  • Use a background that is plain and dark.
  • Good lighting is important. Use natural lighting if possible, or consider using flash as well as your room lights.
  • Take at least one photo that shows a wide area of your body, so that we can see which parts of you are affected.
  • Take at least one close-up photo. Don’t go so close that the picture is out of focus.
  • For problems like moles, put a ruler or coin on the skin close to the mole so that we can see how large it is.
  • Give your camera time to focus on the right spot. If your camera is too close it will be unable to focus.

Any files you attach may be stored on your medical records.
